- Plan 1 - All employees - One times earnings
- Plan 2 - All employees - Two times earnings
- Plan 3 - All employees - Three times earnings (Not available with less than 10 employees)
- Plan 4 - All employees - Flat $10,000
Group Term Insurance With AD&D
Plans are subject to the following maximums:

Benefits reduced by 35% at age 65.
